Complex care coordination provides a single point of contact for care, information and support for your child, when and where you need it.

Looking after a child or young person with a complex medical condition is difficult, especially when there are many people involved in the child’s care.

Care coordinators, or care navigators, can guide and link you to the different services your child needs, to help you feel confident with your child's care. They work with you to decide how long you may need the support for.

Each health service has a different way to enrol children and young people in complex care coordination. The service at the children’s hospitals in Sydney is the KidsGPS service.

• Ensuring communication between the different teams, services and you, including your child’s GP, specialists, hospitals, private health providers, social care services, school and allied health professionals
