About Complex Care Coordination
Complex care coordination provides a single point of contact for care, information and support for your child, when and where you need it.
Looking after a child or young person with a complex medical condition is difficult, especially when there are many people involved in the child鈥檚 care.
Care coordinators, or care navigators, can guide and link you to the different services your child needs, to help you feel confident with your child's care. They work with you to decide how long you may need the support for.
Each health service has a different way to enrol children and young people in complex care coordination. The service at the children鈥檚 hospitals in Sydney is the KidsGPS service.

How the service helps you and your child
- Ensuring communication between the different teams, services and you, including your child鈥檚 GP, specialists, hospitals, private health providers, social care services, school and allied health professionals
- Helping you understand the different parts of the health and social care systems, and how they work together for you
