We hold a bimonthly online webinar exploring how to prepare for healthcare transition.

The aim is to help you understand self-management principles, how to get ready for transition and how transition to the adult system will work.

The session is for young people, and parents/carers of young people, who:

  • receive care, or have received care, with the Sydney Children's Hospitals 星空体育, and
  • are in the early stages of transitioning to adult services or aged between 14-17 years old.
