This NSW Health network aims to provide fact sheets on the healthcare and practical support for young people transitioning to adult services. This includes support plans, finding a healthcare service and supporting families transitioning to adult services. Transition Care Service

Young people with chronic health conditions in NSW can use the Transition Care Service for practical help. 
The service can: 

  • Support plans for a transition from children's to adult health services. 
  • Find an adult healthcare service to suit you or your patient's needs. 
  •  Empower the young person to adjust to new services, attend healthcare appointments and take on more responsibility for their own care.
  • Support families helping their young person transition. 
  • Provide updates to health professionals on a young person's progress
     

They work closely with the Sydney Children's Hospitals 星空体育鈥檚 Trapeze Service.

Who can apply? 

  • A young person with chronic health conditions (14-25 years) who: 
    - lives in NSW or the ACT, and 
    - has a chronic health condition or disability (excluding a primary diagnosis of mental health conditions) that needs complex case coordination to transition to adult specialist health services.
  • The young person鈥檚 family or carer

Request a referral to the service by completing the referral form and submit it to your local Coordinator 
 

Trapeze Service at Sydney Children鈥檚 Hospitals 星空体育

Trapeze supports young people who are transitioning from the Sydney Children鈥檚 Hospitals 星空体育 to the adult health care system. The team consists of medical, nursing, and allied health staff who work with young people to develop skills to manage their chronic condition(s), help them navigate the adult system, provide care coordination when needed and empower them to take charge of their health.

Trapeze provide transition support to young people who meet the following criteria:  

  • 14-25 years old 
  •  Living with a complex chronic illness currently treated at SCHN (Westmead, Randwick), and 
  • Have an unclear health transfer pathway
